Remember, this is not clinical advice and cannot replace a diagnosis. Results will simply show how you compare to a sample of self-selecting participants. 

1. 

Do you consider yourself to be a risk-taker?

2. 

Do you think that greater pleasure is tied to greater risk?

3. 

Do you commit to quitting an activity but find that you keep coming back to it?

4. 

Do you want to stop this behavior that you struggle to regulate? Do you see a problem with continuing?

5. 

Do you find that you self-medicate with certain activities to relieve stress?

6. 

Do you have strong feelings associated with completing certain activities, as if they are an itch that needs to be scratched?

7. 

Do you ever get in trouble at work or school because of the actions you’ve taken (or not taken) as a result of your engagement in this activity? Perhaps you failed to complete an assignment because you chose to drink, for example.

8. 

Have you gotten into legal trouble related to the activity? Perhaps this has happened more than once.

9. 

After a stressful day at work or school, do you find yourself thinking only about engaging in that activity, perhaps thinking it is the only way you are going to find relief from your anxiety, stress and fear?

10. 

Do you recognize how your life suffers because of your commitment to an activity? Even when you recognize this, do you still find it hard to stop engaging in that activity?

11. 

If you can't engage in a specific activity for a period of 24 hours, do you feel pain, anxiety, frustration or suffer a panic attack? Do you have physical complications resulting from not using it?

12. 

Has anyone ever expressed concern with you about your engagement in this activity, either the frequency or the activity itself?

13. 

If there is an activity you struggle to regulate, do you feel pleasure or a sense of relief when you engaging in using it?

14. 

Do you feel like you need to hide the people who you usually engage in this activity with you from your family and friends and keep your lives separate?

15. 

Do you find yourself thinking about an activity, be it an stress-relieving activity or any kind of substance use regularly? Do you think about this activity at least one or more times a day?

16. 

Have you ever become frustrated due to something interfering with your plan to partake in an activity you engage in often?

17. 

Have you developed a health problem that seems to stem at least in part from this activity?

18. 

Do you avoid openly admitting to how often you use this substance/participate in this activity with friends and family because you don’t want people to know how much you do it?

19. 

Do you lie to other people about your activities or behaviors, which could indicate that you are ashamed of them?

20. 

If you’re running out of a substance or won't have access to a particular activity or you know you cannot engage in that activity for a few hours or days, do you get angry and frustrated?

21. 

Do you have trouble sleeping because you are thinking about the activity or substance?

22. 

Are you not taking care of your health the way you would like to, such as eating healthy meals, getting exercise, maintaining personal hygiene and getting sleep?

23. 

Do you ever ask anyone else to supply a substance for you because you are worried that someone will think you are buying too much or, perhaps, you cannot get more?

24. 

Are you facing financial difficulties as a result (at least in part) of your engagement in this activity? It could be due to missing rent or mortgage payments to ensure you have enough of the substance or engaging in gambling away your food money.

25. 

Do you ever take risks to get the substance or participate in an activity, even when you know you should not? Perhaps that includes driving on a suspended license or ducking out of work to get the relief you need?
