Starting early gives us time to identify the right follow-up services, coordinate appointments, and address any barriers before treatment ends. You’ll leave here with a clear plan and the support needed to succeed.
What’s Included in a Discharge Plan
Every plan is individualized to your unique clinical, emotional, and physical health needs.
Your discharge plan may include:
- Referrals to a psychiatrist or therapist
- Recommendations for an Intensive Outpatient Program (IOP)
- Medication management instructions
- Follow-up care for physical health concerns
- Recommendations for continued services
- Health education and ongoing recovery resources

Your Role in Planning and Establishing Connections Before Leaving Treatment
You play an active role in discharge planning by selecting therapists, choosing providers near your home or work, and identifying services that best fit your schedule.
As discharge approaches, you will participate in follow-up meetings to finalize appointments and review all aspects of your transition.
Before you leave, we confirm that appointments are scheduled and that you know how to access the providers and services outlined in your plan, helping create a smooth transition into the next stage of recovery.
What Happens if You Leave Early?
If you leave treatment before completing the program, discharge support continues. We provide referrals and resources in your local community to help you continue care after discharge.
While some individuals may choose not to use these resources, guidance and information are offered to help you reconnect with treatment whenever you are ready.
Family and Support System Involvement
Recovery is often strongest when you have a reliable support system. When appropriate and with your consent, family members and other trusted support persons are included throughout treatment and in discharge planning.
Families may participate in joint therapy sessions, visit the campus for meetings with you and your treatment team, or attend phone or virtual sessions when in-person participation is not possible. These conversations educate loved ones about the recovery process and provide them with tools to offer meaningful support after treatment.
By involving families early and maintaining open opportunities for collaboration, FHE Health helps build a stronger support network that can contribute to sustainable recovery after discharge.

At FHE Health, discharge planning is an ongoing, collaborative process rather than a checklist completed just before you leave. Throughout treatment, clients, clinicians, families, and other members of the support system work together to create a plan that may include:
- Continued behavioral healthcare, such as outpatient therapy, psychiatric care, or a step-down level of treatment.
- Medical and medication support, including follow-up appointments and medication management.
- Recovery resources, such as peer support groups, alumni services, and community-based programs.
- Practical planning for housing, transportation, work, school, and other daily responsibilities.
- Personalized recommendations based on your diagnosis, progress, goals, home environment, and available support.
Because these conversations begin early and continue throughout treatment, you leave with more than a list of referrals. You have a practical, individualized plan for transitioning into the next stage of recovery with greater confidence and support.
